Chapter Event Support Request Form
This form is required for any ACRP chapter requesting event support for ACRP-approved contact hours, assistance with contact hour management, and/or registration support.
Depending on your selections in the Required Information portion of the form below, you will be asked to complete up to 3 sections:
- Section I - Request for Approval for Contact Hours
- Section II - Request for ACRP Contact Hour Management
- Section III - Request for Online Registration Support
Chapters are strongly encouraged to plan ahead for upcoming events. To allow for ample time for approval and promotion of your event, please submit your event request and event materials a minimum of one month prior to your event date. Once submitted, please allow at least 2 weeks for review and processing.
